PAYMENT OPTIONS
All payments must be made in U.S. Dollars. Checks must be drawn on a U.S. Bank in U.S. Dollars and must be made payable to ASC 2008. Accepted credit cards: Visa, MasterCard, and Discover.

To qualify for the early registration fee, your payment - not your payment commitment - must be postmarked by July 7, 2008.
REGISTRATION CHANGES
Requests to change or modify a conference registration must be made in writing and emailed or faxed to Centennial Conferences (contact information below).
CANCELLATION REQUESTS
For cancellations, pre-paid registration fees (except short course fees) are refundable less a U.S. $95 processing fee if notification of cancellation is received in writing by Centennial Conferences by July 18, 2008. Cancellations after July 18, 2008 will result in forfeiture of the registration fee, however, a substitute registrant will be accepted.
PRE-REGISTRATION DEADLINE: AUGUST 1, 2008
After August 1, 2008, registrations will only be accepted at the on-site registration desk at the Hyatt Regency Hotel in Chicago. Pre-registration (including online registration) will be closed at midnight, August 1st, MST.
